Physical Therapist

As a Physical Therapist with L&C, you will have the opportunity to work one-on-one with patients in the comfort of their own homes, helping them improve strength, mobility, balance, endurance, safety, and independence. You will work closely with patients, families, caregivers, physicians, nurses, and other members of the healthcare team to develop and provide individualized care based on each patient's needs and goals.

What You’ll Do

• Complete comprehensive physical therapy evaluations and assessments in the patient's home.
• Develop individualized treatment plans based on physician orders, patient needs, and functional goals.
• Provide skilled physical therapy interventions including therapeutic exercises, strengthening, gait training, balance activities, transfer training, mobility training, endurance activities, and functional training.
• Assess patients for fall risks, safety concerns, changes in functional status, and the need for assistive devices.
• Educate patients and caregivers on exercises, mobility, positioning, safety, fall prevention, and strategies to promote independence.
• Monitor patient progress and adjust interventions as appropriate.
• Communicate changes in patient condition and progress with physicians and other members of the interdisciplinary team.
• Participate in care coordination with nursing, speech therapy, home health aides, physicians, and other healthcare professionals.
• Provide documentation that accurately reflects skilled services provided, patient response, progress toward goals, and continued skilled need.
• Assist patients in achieving their highest practical level of function and independence while remaining safely in their homes.
• Help prevent avoidable complications, falls, hospitalizations, and decline in functional status through appropriate skilled intervention and patient education.
• Maintain compliance with Medicare, Kansas state, and agency requirements.

What We’re Looking For

• Current Kansas Physical Therapist license in good standing.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently while also being an active member of a healthcare team.
• Strong organizational and documentation skills.
• Dependable, compassionate, and patient-centered approach to care.
• Valid driver's license, reliable transportation, and ability to travel to patient homes within the agency service area.
• Previous home health experience is preferred but not required.
