Faith Home Health & Hospice is hiring a licensed Physical Therapist or a Physical Therapy Assistant to support patients in the Wichita, KS area and surrounding communities. This position is a PRN role.

Job Summary

As a Physical Therapist, you will provide skilled in-home therapy services based on individualized treatment plans, working collaboratively with physicians and interdisciplinary team members to maximize patient independence, safety, and quality of life.

What You'll Do

• Implement and modify individualized treatment plans in collaboration with interdisciplinary team members.
• Maintain timely and accurate clinical documentation, including evaluation reports, daily notes, progress updates, and discharge summaries.
• Provide education to patients and families regarding safe mobility techniques, adaptive equipment, fall prevention, and home exercise programs.
• Communicate effectively with patients, caregivers, physicians, and agency staff to support coordinated, high-quality care.
• Participate in case conferences, chart audits, and quality improvement initiatives to ensure regulatory compliance and positive patient outcomes.

Qualifications

• Current Kansas Physical Therapist License required.
• Graduate of an accredited Physical Therapy program (APTA or AMA approved).
• Home Health experience preferred.
• Current CPR certification required.
• Valid driver's license, current auto insurance, and reliable transportation.
• Demonstrated ability to work independently while maintaining strong communication with the care team.
