Job Summary:

As a Physical Therapist, you will provide in-home therapy services based on individualized treatment plans, collaborating with physicians and interdisciplinary teams to ensure optimal patient outcomes.

What you will do:

• Complete initial patient evaluations within 48 hours of referral and conduct re-evaluations every 30 days to ensure appropriate plan of care.
• Develop, implement, and modify individualized treatment plans in collaboration with physicians and interdisciplinary team members.
• Maintain timely and accurate clinical documentation, including evaluation reports, daily notes, progress updates, and discharge summaries.
• Provide education to patients and families on safe mobility techniques, proper use of adaptive equipment, and individualized home exercise programs.
• Communicate effectively with patients, caregivers, physicians, and agency staff to support coordinated, high-quality care.
• Participate in case conferences, chart audits, and quality improvement initiatives to ensure compliance with regulatory standards and enhance patient outcomes.

Qualifications

• Licensed Physical Therapist in Kansas.
• Graduate of an accredited PT program (APTA or AMA-approved).
• Preferred: 2+ years of clinical experience, prior home health experience
• Valid driver’s license and reliable transportation.
• Current CPR certification and compliance with required documentation.
