Position summary
The Occupational Therapist provides occupational therapy services to patients in accordance with the established plan of care and consistent with the philosophy, objectives, and policies of the agency. This role partners closely with physicians, nursing staff, caregivers, and other clinicians to help patients improve function, safety, and quality of life in the home setting.
What you will do
- Evaluate patients’ functional levels and help develop treatment plans and goals in collaboration with the physician and care team
- Complete initial evaluations within 48 hours of referral and complete re-evaluations every 30 days, as appropriate
- Provide occupational therapy services based on the established plan of care
- Educate patients and families on therapy programs, equipment use, and safe patient management
- Observe, document, and report changes in patient condition or care needs
- Revise treatment plans and communicate changes to the physician as needed
- Prepare accurate clinical documentation, including evaluations, progress notes, and summaries
- Collaborate with nursing staff and other disciplines to support coordinated patient care
- Participate in chart reviews, case conferences, peer review, QAPI activities, and staff education as requested
- Communicate clearly and professionally with patients, families, caregivers, and agency staff
- Complete all duties accurately and in a timely manner
- Perform other duties as assigned
Qualifications
- Licensed as an Occupational Therapist in the State of Kansas
- Graduate of an OT curriculum approved by the American Occupational Therapy Association or the Council of Medical Education and Hospitals of the AMA
- At least 1 year of Occupational Therapist experience in a clinical care setting preferred
- Prior home health experience preferred
- Valid driver’s license and reliable transportation required
- Current TB test, CPR certification, and other required documentation
